Join our team as a Case Navigator!! The Case Navigator is responsible for both navigating homeless individuals and families referred to the program to match them to the appropriate housing opportunity and case managing these participants to successfully complete the program. Housing Navigation includes screening participants using an assessment workflow through the Homeless Management Information System (HMIS) and will accompany clients from the housing referral through the lease-up process. The position will also collaborate with landlords and community partners. Responsible for working with clients participating in the Rapid Rehousing Program, coordinating/updating referral services, and providing input regarding complex cases. Also responsible for maintaining client files in accordance with agency policies, inputting client information in a designated database(s),and gathering and reporting program metrics.

  • You have an Associate Degree; or
  • You have two (2) years of experience in lieu of education
  • You have one (1) year experience in case management
  • You have a valid Texas Driver’s License

  • You have experience conducting outreach and presentations.
  • You are flexible and effective team player, able to adapt to changes and a busy workplace environment.
  • You have the knowledge and experience with housing issues and available resources.
  • You have the experience and knowledge with the challenges of and resources for those at-risk of homelessness.
  • You have the knowledge of homelessness and related issues, and poverty-related issues.
  • You have the ability to work with diverse communities.
  • You are excellent organizational skills and an ability to self-motivate.
  • You are excellent written/verbal communication and people skills.
  • You have computer literate – experience with MS Office: Word, Excel, Outlook, and using internet.
  • You have the ability to work under pressure, address multiple priorities and meet deadlines.
  • You have the ability to identify key issues in problem solving and implement effective and creative solutions.

 

 

What you Deliver

 

  • You will screen homeless persons for housing eligibility using the Coordinated Access assessment tool. Provides client referral to the appropriate housing intervention and potential housing sites. Pulls housing referral from HMIS and begins the next step process to enrollment. Assists individuals that are homeless in gathering all documents necessary to complete a housing program enrollment. Accompanies clients to all housing appointments and serves as clients’ primary advocate during the housing search and selection process and prior to transfer to the housing case manager.
  • You will assesses individuals and families for strengths and barriers, including employment, behavioral health, criminal history and substance use and refer household to appropriate levels of housing and rental assistance. Assists individuals and families with establishing and meeting housing and related goals, and identifying appropriate steps towards achieving goals, including modifying goals as needed, and evaluation of progress. Carries a caseload of individuals and families, which may include home-based case management.
  • You will maintain precise and accurate documentation of case management services, including client files and entries into the program databases.
  • You will attend Housing Assessor, Navigator, HMIS and other applicable trainings, as assigned, in order to fully utilize the Coordinated Access system accurately and implement the navigation portion of the program effectively.
  • You will coordinate with a variety of outside providers, as necessary, to meet client’s needs and avoid duplications of services. Provides community resource referrals for clients.
  • You will perform other duties as assigned.
