The Houston Health Department seeks dynamic candidates that will fill the role of the Senior Health Education Coordinator for Chronic Disease Health Education Programs. The Senior Health Education Coordinator is responsible for planning, implementing, and evaluating community-based and clinic patient health education and health promotion initiatives. Essential duties include:

Workdays & Hours: Monday - Friday, 8:00 a.m.- 5:00 p.m. (Flexibility includes weekends & evenings as needed)*

*Subject to change Essential Job Functions includes but not limited: 

  • Conduct health education, patient education, and health promotion programming including providing specialized chronic disease health information, knowledge, and skills to various target populations.
  • Apply appropriate health behavior theories and evidence-based models to plan, organize, implement, and monitor health education and health promotion programming in clinic and community-based settings. 
  • Maintain and nurture close working relationships with internal staff, community-based and private sector organizations to coordinate and perform health education and health promotion activities.
  • Facilitate culturally and linguistically appropriate content for health education materials, visual aids, presentations, interactive demonstrations for one-on-one health coaching, group classes and social media settings. Content includes but is not limited to hypertension prevention and management, obesity prevention and weight management, tobacco use prevention and cessation, stress management and lifestyle modification, and national health observances.
  • Maintain subject matter knowledge and professional development in the discipline of Chronic Disease prevention and self-management. 
  • Perform evaluation activities including but not limited to applying and implementing data collection instruments, collecting and managing data, maintain sign-in sheets and logs, preparing reports, and participating in quality assurance and quality improvement activities. 
  • Maintain health education supplies, inventory, and assets.
  • Utilize various inter-disciplinary, communication and marketing techniques to promote and disseminate actionable health and wellness information and advocate for health equity.

WORKING CONDITIONS

The position occasionally requires stooping or bending. Occasional very light lifting, such as three or four reams of papers or books (up to 20 pounds or an equivalent weight) may be required. There are no major sources of discomfort, i.e., essentially normal office environment with acceptable lighting, temperature and air conditions. 

MINIMUM REQUIREMENTS

EDUCATIONAL REQUIREMENTS:

Requires a Bachelor's degree in Education, Behavioral Science, or a closely related field such as Public Health.


EXPERIENCE REQUIREMENTS:

No experience is required.


Directly related professional experience in Public Health, Education or a closely related field may be substituted for the education requirement on a year-for-year basis

PREFERENCES

**Preference shall be given to eligible veteran applicants provided such persons possess the qualifications necessary for competent discharge of the duties involved in the position applied for, such persons are among the most qualified candidates for the position, and all other factors in accordance with Executive Order 1-6.** 


  • Knowledge in the sub-areas of nutrition, physical activity, diabetes, heart disease, tobacco cessation, and/or stress management are preferred
  • Certified Health Education Specialist (CHES®) or CHES eligible are preferred
  • Health coach, personal fitness, dance fitness instructor certifications or similar certifications are preferred
  • Experience with recruiting participants into health programs
  • Experience using Microsoft Office products including Outlook, Word, Excel, Teams, and PowerPoint or Adobe Suite in generating flyers, materials, correspondence, reports, presentations

GENERAL INFORMATION

None. However, the Department may administer a skills assessment test.

Pay Grade 14


SALARY INFORMATION  SPECIAL FUNDED

Grant and Special funded positions are dependent upon continued available funds. If funding is no longer available, employee may be laid off or transferred. Factors used in determining the salary offered include the candidate's qualifications as well as the pay rates of other employees in this classification.
