Vecino Health Centers is hiring a Community Health Coordinator to lead its Community Health Worker teams on various projects within our clinics' service areas.

The purpose of the Community Health Coordinator position is to provide outreach, health literacy, and navigation services in the community to connect individuals and families within the Vecino HRSA service area (SA) to the services available at Vecino. This position works collaboratively with key partners (such as the UH Community Health Worker Initiative, Fifth Ward Health Collaborative, the Northside Health Collaborative, and Harris County Precinct 2 to cite just a few) to engage community members.  

ESSENTIAL JOB DUTIES & RESPONSIBILITIES 

1.  Implement a recurrent system to deliver health literacy programming and navigation services within Vecino’s 25-zip code service area.

2.   Help increase Vecino’s footprint within its service area.

3.   Map and maintain a comprehensive database of community assets to engage various types of entities and stakeholders in access to care campaigns and health literacy programming including area businesses, faith-based communities, nonprofits, and public entities.

4.   Increase collaborative efforts with community partners to recruit new patients and impart health literacy programming.

5.   Serve as community liaison with entities in the Vecino service area to promote Vecino services and provide navigation services to connect individuals and families to Vecino.

6.   Document, track, and follow through with community requests for health literacy and access to care activities and programming.

7.   Maintain a dashboard to track and assess the results of Vecino’s community health efforts in terms of health literacy offerings and recruitment of new patients.

8.   Supervise temporary and contract staff for ad-hoc community projects.

9.   Responsible for evaluating performance and implementing a continuous quality improvement system to device annual, quarterly, and monthly work plans.

10.   Work collaboratively with Vecino’s Patient Experience Coordinators to increase new patients’ engagement in preventive services and the patient portal.

11.   Work collaboratively with Social Services Consultants to link new patients that may need assistance with Social Determinants of Health and to help map resources available within the service area.

12.   Work collaboratively with the Eligibility Team to help new patients access benefits including enrollment in the Affordable Care Act (ACA).

13.   Work collaboratively with the Director of Program Development to identify new opportunities for collaborative work within the SA.

14.   Work collaboratively with Marketing Consultant to develop appropriate materials for new patients and members of the community.

15.   Work collaboratively with the Health Promotion Coordinator to engage new patients in health-seeking behaviors.

16.   Keep abreast with industry and regional trends related to community engagement, community health work, and access to care.

17.   Participate in the Patient Appreciation Committee to help implement events and efforts to show appreciation to new and existing patients.

18.   Procure in-kind donations to be used as incentives and giveaways in community events and health promotion programming for new and existing patients.

19.   Maintain appropriate recordkeeping for agency reports and accountability purposes.

20.   Other duties as assigned by supervisor, CMO, or CEO.

QUALIFICATIONS, EDUCATION, AND EXPERIENCE

1.  High School Diploma or equivalent required. Associate degree preferred.

2.   Community Health Worker certification required.

3.   Minimum of two years of customer service experience or community-based work.

4.   Minimum of one year of experience in a healthcare setting.

5.   Experience supervising staff preferred.

6.   Bilingual English-Spanish; must be able to speak, read, and write fluently in both languages.

7.   Experience participating and organizing community events.

8.   Excellent organizational and interpersonal skills to establish effective working relationships.

9.   Tactful and respectful.

10.  Impeccable work ethic.

REQUIREMENTS

1.   Ability to sit, stand, bend, and stoop for (long) periods of time

2.   Ability to exert up to 20 pounds of force occasionally/frequently

3.   Ability to work in a stressful environment

4.   Ability to respond to emergency/crisis situations

5.   Exposure to noise

6.   Evenings and weekend work required
