The Veterans Program Case Manager assists Veterans dealing with crises situations, homelessness, substance, and justice-involved issues. Creates and manages a plan of care with Veterans who find themselves in need of assistance, by educating and connecting Veterans (and or family members) with various resources. This role will manage care through community partners and Veteran Affairs to connect clients with physical/mental health, housing, substance programs, and legal resources. Conducts case management for all clients utilizing the 30/60/90 model. 

Duties and Responsibilities:

  • The Case Manager will assess Veteran issues, develop a plan for referrals and utilization of services as needed, consultation with specialists and guide them to any variety of programs with a goal to meet needs for care, including, but not limited to; justice systems, homeless, medical facilities, emergency financial assistance, bereavement services, intake services, etc. This includes exit strategies/plans from jail or diversion centers.
  • Ability to control volatile situations in a professional manner, help Veterans have a positive attitude, and recognize patterns of behavior and self-understanding to focus on following plan in using available resources.
  • Not only develops service plans, but also continues to monitor progress through reassessments with Veteran/family members and other organizations involved in Veteran treatment/care plans. Adjust if necessary; the end goal is to get Veteran back into optimum social functioning and self-sustaining to the best of their abilities.
  • Build rapport with internal and external organizations (Sheriff’s Dept, MCOT, HOT, etc), which we work with to build effective collaborations in getting the best possible outcomes for our Veterans; this is an “eyes and ears” approach to finding community issues where they are and not reacting afterward.
  • Certified Peer to Peer Counselor or similar certificate, highly desirable.
  • Great personality with a "can do" attitude, organized, conscientious work ethic, and genuine desire to care for Veterans.

Education and Experience:

  • Associates Degree is required from an accredited college or university.
  • 2-5 years experience as a Case Manager who can develop and administer a plan for Continuity of Care: working with Veterans
  • It is paramount to be familiar with our Veteran community and specific resources; issues, concerns, traumas, family concerns, organizations, etc. AND 
  • The ability to build mutual respect, develop effective relationships, and work effectively with co-workers and the community at large, specifically Veterans and family members.
  • Active TX driver’s license is required 

Knowledge, Skills, and Abilities:

  • Case Managers will focus on issues of Veterans; predominantly justice-involved, homeless, substance abuse, and suicide prevention. This will include being proactive, not reactive. 
  • Must be comfortable going to jails, in parks/streets, hospitals, rehabilitation centers, and crisis intervention to find Veterans in need of assistance, guidance, and mentorship to include case management and coordination of care with community partners.
  • Extensive knowledge of Veteran and community resources; including VA, state, county, and nonprofit organizations, and ability to learn and apply policies and procedures pertinent to the division and the agency.
  • Receive training by the staff and community, to remain at the forefront of program development of effective care within our community, in which we can coordinate partnerships to help Harris County Veterans.
  • Must be computer literate with MS Office (Word, Excel, PPT, etc.), internet research proficiency, and able to learn other software as required

  • Bachelor's Degree in Case Management, Social Work, Counseling, Human Services or related field from an accredited college or university
  • Peer to Peer Certification highly desirable

  • Based on 26 Pay Periods
  • Will be discussed during the departmental interview

Work Environment:

  • The work is mostly in an office or outside environment
  • Will be occasional local travel to complete job functions, training, meetings, Veteran crisis response/follow-up, etc. 
  • No special safety precautions are required

Employment is contingent upon passing a background check and drug screen.
