Under the supervision of the Associate Director, the Community Health and Wellness Program Manager will oversee Public Health’s chronic disease portfolio including Diabetes, Asthma, Obesity, and Tobacco Cessation. The manager is responsible for improving health outcomes through innovative evidence-based programs and community partnerships. The incumbent will also serve to procure relevant grants, provide operational oversight, and content expertise.


Operational

  • Actively participates and/or leads key programmatic activities such as the setting of program goals and priorities, implementing strategies, evaluating outcomes, and ensuring coordination of Community Health activities.
  • Develops reviews and updates policies and procedures for the CDP Section in coordination with Quality. Prepares reports and correspondence, including statistical analysis of CDP activities.
  • Develops, supervises, and/or coordinates the implementation and evaluation of CHS grant compliance.
  • Oversees ancillary support functions for CDP.
  • Oversees community outreach services and activities.
  • Responsible to produce evidence-based community health outcomes
  • Budget management including grant allocation

Clinical/Community Resources

  • Coordinates/collaborates with partners (internal and external) to establish and maintain professional networking relationships.
  • Responds appropriately and timely to inquiries made by internal and external partners/collaborative organizations.
  • Serves as the liaison between CDP and community partners/collaborative organizations.

Other

  • Other duties as assigned, including special tasks involved in responding to an emergency/disaster event.

Requirements

Education Requirements:

  • Master’s Degree in Health Administration, Public Health, Business, or related field

Licenses/Certifications Required:

  • Must have a valid Driver’s License (TX upon hire) and personal transportation for periodic travel.

Experience Requirements:

  • Two (2) years of experience in health care management
  • Thorough understanding of the principles of public health practice.
  • Grant Programming

Computer Proficiency:

  • Intermediate skills in the Microsoft Office Suite

Special Skills:

  • Ability to provide technical assistance, training, and coaching to support staff.
  • Must be highly motivated and possess the ability to assist with communicating the divisional goals and objectives to personnel, including peers and contract personnel
  • Knowledge of the principles, tools, and strategies used in change management, stakeholder engagement, assessment, planning, and evaluation. 
  • Must have skills in facilitating group processes

  • Employee must be in good health and physical condition sufficient to permit the full performance of the duties of this position.

Preferences

Experience Preferred:

  • Direct supervisory experience in an administrative or health-related setting.

Computer Proficiency:

  • Proficient in Microsoft PowerPoint and Access desirable.

Special Skills:

  • Bilingual abilities in English and Spanish.
  • Basic experience with project involvement and/or management.
