Clinic Services Coordinator is responsible for the coordination and overall operations of healthcare services provided through WFDC Clinic to ensure outstanding patient care.

 

Job Responsibilities:

1.    Manage daily operations of WFDC Clinic, including; overseeing providers and volunteers, coordinating healthcare providers’ schedules, and ensure smooth patient flow throughout the day.

2.    Coordinate and collaborate with other healthcare providers to improve physical and mental healthcare of WFDC Residents.

3.    Assist WFDC Residents in identifying and securing their “Medical Home” before transitioning from the WFDC.

4.    Identify healthcare needs of WFDC Residents and coordinate weekly seminars educating and equipping Residents for improved care.

5.    Maintain appropriate reports and submit all pertinent data as directed by the Manager of Resident Services while maintaining all required confidentiality guidelines.

6.    Identify and build new collaboration with other homeless service providers to create a continuum of care to WFDC Residents.

7.    Ensure healthcare providers adhere to WFDC policies and procedures, and services are aligned with SOH mission and guiding principles.

8.    Coordinate the provision of healthcare related education to WFDC Residents.

9.    Assist the Manager of Resident Services II in developing, recording, and implementing of WFDC Clinic policies and procedures.

10.  Act as a liaison among healthcare providers, staff and patients.

11.  Stay apprised of mental/physical healthcare requirements and standards of care and provide such information to staff as needed.

12.  Provide appropriate reports, follow-up information and report all unsafe conditions and behaviors immediately to Management (i.e.-significant event reports and work orders).

 

               

Other Job Responsibilities:

1. Participate in Ministry and prayer activities as appropriate.

2. Other duties as assigned.

Qualifications:             

Education, Skills & Experience: BS in Social Services, Social Work, or equivalent experience in related field. 3 years previous experience in a healthcare environment or other social   work setting, including chemical dependency agencies. Previous experience working

with mental health professionals and clients. Excellent written, documentation and communication skills. Ability to maintain confidential client files. Valid Texas Driver’s License with acceptable driving record and current liability insurance.

Good reading, writing, time management and ability to multitask. Detail orientated with strong communication skills. Must be able to clearly communicate information about residents, policies, and procedures. Good judgement in dealing with the residents and confidential matters. Ability to problem-solve, work well without supervision. Ability to communicate and work cooperatively with employees, managers, residents, volunteers and outside organizations that interact with Star of Hope. Professional appearance and behavior.  

Be able to work respectfully, professionally and in a Christ-like manner with everyone, able to maintain required residents/staff boundaries and possess a willingness to serve the Lord in a ministry that deals with homeless men, women and children.

Equipment Used: General Office Supplies (Calculator, Telephone, Stapler, etc.), laminator, Computer Equipment (Monitor, Keyboard, etc.), and Copier/Fax/Printer, Software: Microsoft Word, Excel, PowerPoint, Outlook, Sphere (Inventory supply) and SharePoint.

Work Environment: Normal business hours, office environment, occasional weekend or evening events. Occasional visits to program sites may be required. Work schedule may be subject to change to meet the business needs of the mission. Exposure to infectious diseases, poor hygiene, drug usage, verbal and physical abuse and other emergency situations relative to the homeless population is normal. Jeans with no holes/fraying may only be worn on Fridays and only as long as a Star of Hope shirt (T-shirt, polo-style or sweat shirt) is worn.

Relations with Others: Frequent face to face discussions with individuals, immediate peers, immediate manager and direct reports. Frequent use of electronic mail. Frequent contact with residents and volunteers. Frequent contact with peers and managers in other departments. Occasional telephone conversations and written letters and memos. 

Physical Requirements: Medium work with lifting of up to 30 pounds. Constant standing, talking, hearing, field of vision and depth perception. Frequent stooping, kneeling, crouching, reaching, walking, use of fingers, repetitive motions, handling, and color vision. Occasional climbing, crawling, sitting, pushing, pulling, feeling, lifting, smelling, and driving.
