The Medical Records Clerk processes medical record and billing statement requests, following Baylor policies and HIPAA guidelines. Communicates with patients and third parties regarding status of requests and document requests. Completes affidavits and questionnaires, and uses excel for daily productivity. Computer skills are necessary, including Excel, Word, and knowledge of file sharing via email or uploading to portals.

Job Duties

  • Assists with all aspects of Release workflow, including: opening and sorting mail, emails, and mychart requests, scanning and starting/ completing medical and billing statement requests within Epic. 
  • Prioritizes STATs for patient care. 
  • Reviews requests to ensure HIPAA guidelines are followed. 
  • Reports any breaches to Supervisor/Compliance Department. 
  • Assists with any requests to amend patient records and communicates and follows up with physicians to ensure timeliness of responses. 
  • Assists compliance with any OCR complaints. 
  • Provides Baylor Legal department records as needed. 
  • Keeps a productivity log daily.

Minimum Qualifications

  • High school diploma or GED.
  • Two years of relevant experience.

Preferred Qualifications

  • Associate Degree Applied Science Major in Health Information Technology.
  • Healthcare/Medical records experience.
  • Medical Terminology.
  • Extensive knowledge of HIPAA.
