Under the supervision of the Health Impact Team (HIT) Tobacco Program Team Lead, and with oversight and guidance from the Nutrition and Chronic Disease Prevention Manager, the HIT Program Specialist II is responsible for assisting with the development, implementation, and continued support of the Tobacco Cessation and Youth Vaping Prevention initiatives in Harris County. They will collaborate amongst the team to continually improve programs geared toward tobacco cessation and prevention.


Outreach/Admin/Education 

  • Provides engaging, evidence-based, individual and group tobacco cessation and prevention education that encourages self-management skills for clients and program participants 
  • Adapts education and counseling to specific economic, social, cultural, and age-appropriate need of patient to enhance compliance and health outcomes 
  • Plans and implements health activities that align with a variety of age groups and demographics 
  • Assists in the development and creation of tobacco and vaping cessation and prevention curricula, trainings, and presentations to meet the needs of the target population 
  • Plans and conducts trainings for healthcare personnel as well as the general public 
  • Assists in the development, implementation, and support of the mobile events and initiatives 
  • Participates in community health events in collaboration with various interagency departments 
  • Develops relationships and partnerships with prospective community partners within Harris County 
  • Types letters, memorandums, agreements, and flyers as assigned 
  • Registers, updates, and clinically documents in client records in Epic per departmental standards 
  • Assists with database creation and management performing data entry extraction, tabulation, reporting and distribution for the DSRIP Program as requested 
  • Records and submits required programmatic and departmental reports documenting work activities according to established protocol

Other 

  • Responsibilities include other duties as assigned, including special tasks involved in responding to an emergency event 
  • Candidate in this role will be expected to assist with other aspects of the program when needed

Requirements

  • Bachelor’s Degree from an accredited college or university
  • One (1) year of experience working with the youth (community centers, schools, youth camps, after school programs, rehab centers) 
  • One (1) year of experience educating and/or providing health related workshops in diverse communities 
  • Intermediate proficiency with MS Office Software such as MS Word, Excel, Teams and Power Point 
  • SharePoint Experience
  • Valid Driver’s License (Texas upon hire) along with a good driving record required for regular travel between locations when needed

Preferences

  • Bachelor’s degree in Health-Related Field
  • Bilingual in English and Spanish

  • Commensurate with Experience
