Position Description

Under the supervision of the Chronic Disease Manager, along with some oversight from the Senior Manager of Chronic Disease Prevention, the Program Support Coordinator will manage the day-to-day operations of the Healthy Aging Program, organize events, develop relationships with community partners, facilitate educational workshops, and coordinate home assessments and modifications and all aspects of the Healthy Aging Program.


Program Support

  • Coordinates and schedules Chronic Disease Self-Management, Diabetes Self-Management, Chronic Pain Self-Management, A Matter of Balance, and other educational workshops in various sites across Harris County
  • Establishes community partnerships
  • Recruits new sites for workshops
  • Organizes meetings and records meeting minutes
  • Orders project supplies and incentives as needed
  • Follows up on and schedules home assessments and home modifications as necessary
  • Maintains project documentation and reports
  • Troubleshoots any issues that may arise from a programmatic standpoint

Team Management

  • Provides direct oversight of healthy aging team, including the case manager, clinical support specialist, and program facilitator
  • Directs work activities of project volunteers and interns including coordinating and scheduling project activities
  • Holds weekly team meetings to provide and receive updates on program objectives
  • Schedules regular touchpoints with direct management and supervisory team 

Administrative Support

  • Participates as an integral member of the administrative team to assure high quality service to community partners, program participants, and coworkers
  • Attends and participates in staff meetings Other Responsibilities

Requirements

  • Bachelor’s degree from an accredited university in Public Health, Business, or a health-related field
  • One (1) year experience working in a health care setting with responsibilities in public health assessment, planning, and/or evaluation
  • Must have thorough understanding of the principles of public health practice
  • Must have knowledge of the principles, tools, and strategies used in coordinating public health programming
  • Basic skills using MS Office software such as MS Word, Excel, and PowerPoint
  • Valid driver’s license (TX upon hire) and reliable personal vehicle. Will be required to travel locally within Harris County to various workshop sites.

 Additional Requirements

  • Lay Leader Training within one year of employment
  • Certified Master Trainer within two years of employment

Preferences

SALARY:
