Position Description

The Health Equity Coordinator facilities and supports the work plan activities and outcomes primarily funded through Ending the HIV Epidemic: A Plan for America — Ryan White HIV/AIDS Program Parts A and B (EHE). The Health Equity Coordinator will collaborate with the HIV Prevention and Treatment stakeholders and clinical providers, and coordinate and facilitate community and patient engagement events.
 
Development: 

  • Coordinates and/or implements strategies to assess health inequities and their root causes (e.g. determinants of health) that contribute to differences in health outcomes across various populations. 
  • Identifies and gathers qualitative and quantitative data, and other information (e.g. health-related literature, policy briefs, legislative and regulatory documents, etc.). 
  • Synthesizes information and develops reports or recommendations to guide planning, practice, evaluation, and policy development to advance equity. 
  • Convenes multi-disciplinary Case Review Team to examine service delivery gaps and opportunities in vulnerable populations where linkage and viral suppression goals are unmet. 
  • Develops technological solutions and systems for continuous improvement of programs. Uses technological solutions to collect, store, and retrieve data. 
  • Implements programs improvements for transportation services that utilizes ride sharing providers such as Uber and Lyft. 
  • Launches community HIV treatment services awareness campaign. 
Community Engagement: 
  • Engages EHE funded Primary Care providers in Technical Assistance to develop clinic protocols for Test and Treat (Rapid Start). 
  • Serves as RWGA EHE Liaison with Coordinator of the Houston Health Department process for jurisdictional EHE plan. 
  • Coordinates with local, state, federal and private partners for the EHE plan. 
  • Communicates with community members and internal/external stakeholders. 
Support: 
  • Completes special projects as assigned, working on a wide variety of financial, administrative, professional, management and community issues and problems of medium complexity. 
  • Coordinates preparations, assembly, and delivery of proposal documents and provide additional information as required by funding sources. 
  • Compiles data, produces information, and interprets results through conclusion or recommendation formulation. 
  • Analyzes and evaluates programmatic public health data. 
  • Presents findings and compiles reports, develops special documents or publications as assigned. 
  • Represents supervisor at meetings and functions generally to gather information. 
  • Participates in evaluation and design of workflow, processes, various operations systems, etc. and create recommendations to improve performance. 
  • Handles confidential information. 
  • Performs data storage and de-identification procedures in accordance with institutional and national policies to ensure maintenance of confidentiality and privacy. 
Other Responsibilities: 
  • Performs other duties as assigned including special tasks involved in responding to an emergency event.

Requirements

  • High school diploma or equivalent. 
  • Four (4) years of experience within the HIV community organization or advocacy (i.e. Lived HIV experience navigating healthcare systems, organizing HIV community outreach, legislative advocacy for HIV-related support). 
  • Must have valid Driver’s License (TX upon hire) and reliable personal vehicle. 
  • Local travel required 20% of the time. 
  • Working knowledge of quality management and quality improvement methodologies. 
  • Knowledge of philosophy, principles, and practice of health care delivery to persons living with chronic illnesses such as HIV, substance abuse, or mental illness. 
  • Intermediate competence in MS Office, database, and spreadsheet software applications. 
  • Ability to work with diverse communities (e.g., communities of color, gay/lesbian, transgender, substance abusers).

Preferences

  • Bachelor's or Master’s degree in nursing, social work, or public health or behavioral health degree related field. 
  • Texas Licensed Social Worker. 
  • One (1) year of experience in the HIV field. 
  • One (1) year of experience with quality-of-care chart abstraction for social services clients. 
  • Intermediate competence in SPSS or comparable data analysis software used to analyze complex health-related databases. 
  • Bilingual (English/Spanish).
